Rapid Assessment Community Evaluation (RACE)

The RACE team of nurses assess aged care residents and community rehab patients to help prevent hospital presentation. The nursing team provides assessment and management of acute medical conditions, where specific management would otherwise require a resident to briefly attend an Emergency Department. The initiative aims to keep residents happy, healthy and in their homes.
